Percentage-Based Billing Fee versus Fixed Billing Fee
Many of our clients are once again receiving solicitations from Revascent, a healthcare practice management company based out of Boise, Idaho. Revascent purchased the MSP billing company Healthcare Process Consulting (HPC) and serves approximately 130 clients. HBS serves over 430 Ohio School Districts. Their latest solicitation is touting a “fixed-fee” billing model versus a “percentage-based” model. Some of our clients have elected a fixed-fee contract with us, but most still prefer the percentage-based contract. There are pros and cons to both methods, and we are always willing to structure our fees using the method that works best for your district.
• A Percentage-based contract is based on the total MSP reimbursement that your district receives. Our standard percentage is 7%, so you pay $70.00 for every $1,000.00 of MSP reimbursement you receive. As we increase your MSP reimbursement, we earn more, but if reimbursement decreases, so does our fee. It gives us the added incentive to get you every dollar you are entitled to receive while making sure that you never owe money back. We don’t get paid until you get paid, which in the case of Final Settlements is two years after the services were provided. The downside of this method is that we cannot put percentage-based fees on the MSP Cost Report.
• A Fixed-fee contract is a flat annual/quarterly fixed rate regardless of what you receive in MSP reimbursement. It is paid for in the year the services are provided, even though much of your reimbursement is paid out two years later in the Final Settlement. When the Cost Report is filed two years later, we can include the fee in the Cost Report, and it is reimbursed at a 50% FMAP. The downside of this method is if MSP reimbursement decreases, the fixed fee was already paid two years earlier.
The choice is always yours as to what billing method you prefer but always make sure that you are comparing “apples to apples” when choosing your billing vendor. HBS does the following:
- Provides user-friendly software with quality and timely customer support.
- Provides support for NPI, CAQH, and Medicaid initial applications and revalidations.
- Provides Random Moment Time Study (RMTS) coordination with ODM and SSG, the RMTS vendor.
- Provides Medicaid Eligibility Rate coordination and completion.
- Completion of the district Indirect Cost Rate (ICR) proposal.
- Provides timely status reporting which is important for maximizing MSP reimbursement.
- Parental Consent monitoring which is critical for program compliance.
- Provides clear, accurate, and convenient training.
- Cost Report preparation without requesting extensions. (Beware some vendors contract out for Cost Report preparation)
- Coordinates completion of the AUP audit.
- HBS represents all districts at State and National conferences.
- Pursues all MSP reimbursement opportunities. (Expansion, MAC, Transportation, Medical Equipment)
